Position Summary

This position is responsible for providing education and support to screening and patients. Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. May participate in some aspects of clinical care. This position exists in various departments throughout OhioHealth system of hospitals and healthcare services.

Minimum Qualifications

Education, Certification, Registration, LicenseBachelor's of Nursing
RN - Registered Nurse
Years of Experience: 3-5 years clinical experience

Applicable clinical knowledge expertise.

Additional Job Information

Would prefer candidate with neurology experience. BSN required. Will involve rotation to weekends (every third). Will consist of coordinating new consults to the appropriate Neurology team, obtaining outside records, communicating with members of the health care team, working with the multidisciplinary team to facilitate the discharge process, providing patient and family education, arranging post discharge follow up and documentation in the EMR.

12 hour shifts or 4 ten hour shifts
