The Investigative Agent/Quality Assurance/Compliance position is responsible for all investigative functions including conducting initial review, investigation, reporting, follow-up and closure activities regarding all assigned incidents of alleged abuse/or neglect, other Major Unusual Incidents (MUI's) and Unusual Incidents (UI's). This role will also support Heinzerling's regulatory compliance and resident quality of care.

Qualifications are as follows:

  • Bachelor's degree from a four-year college or university
  • Two years related experience and/or training working with individuals with disabilities required
  • Must have two years experience conducting investigations preferred
  • Ability to demonstrate understanding of Licensure and Medicaid rules and regulations
  • Strong writing skills for documentation of reports, memos and other company correspondence
  • Detail oriented
  • Excellent organizational skills
  • Ability to effectively communicate in small and large settings
  • Excellent interpersonal skills
  • Ability to work well independently
  • Must have current driver's license with no more than 5 points and proof of auto insurance.
  • Must provide driver's abstract.
  • Must be available for on call responsibilities

Job Responsibilities:

  • Ensures a safe environment for residents
  • Responsible to identify report and investigate all MUI's and UI's in accordance with Heinzerling's policies.
  • Immediately reviews MUI's to ensure the organization has taken all measures necessary to protect the health and safety of the individual and to determine if additional measures are required.
  • Interviews involved persons, including, as appropriate, the individual, witnesses, primary person(s) involved, medical professionals, Heinzerling staff, family, etc.
  • Meets all investigation and reporting requirements for accuracy and timeliness established by the Department of Developmental Disabilities (DODD).
  • Reviews relevant documents relating to incident, alleged victim and primary person involved (alleged perpetrator).
  • Conducts follow up interviews as needed. Obtains written statements from relevant witnesses.
  • Completes investigation case notes and investigative report
  • Analyzes information obtained through MUI investigation and completes an investigation report for DODD.
  • Responds to inquiries from DODD MUI Unit.
  • Reports on MUI's and UI's trends and patterns at Resident Rights monthly meetings.
  • Maintains MUI/UI files. Enters incident reports in database.
