Case Manager - PAR Program (Police Assisted Referral Program)

Cleveland, Ohio - FULL TIME

Beech Brook, one of Northeast Ohio's premier behavioral agencies, is currently hiring a Case Manager that will work within our PAR Program. The Case Manager will be providing services throughout Cuyahoga County. This position will provide community based services to clients referred by CMHA (Cleveland Metropolitan Housing Authority) Police Department. This individual will provide community based referrals and resources to those families that have been identified by the CMHA Police Department as needing intervention due to participating in or being a victim of violence. The ideal candidate for this position will have excellent communication skills, both written and verbal. The individual selected for this role, must have the ability to engage families by phone and through face-to-face contact. The individual chosen for this position will be comfortable with home visits and have strong engagement skills. This person will need experience providing direct service in an urban setting, preferably two (2) or more years' experience. Knowledge of community resources and experience working within a community based setting is needed. Candidates must have a valid OH driver's license with no more than 5 points. A Bachelor's Degree Required; an LSW preferred.

Essential Responsibilities (others may be assigned):

  • Document all contacts with clients.
  • Ability to demonstrate engagement, planning, and intervention with clients.
  • Able to work flexible schedule to meet the needs of the program
  • Able to travel to different community based locations
  • Able to provide backup to other roles
  • Communicates effectively verbally and in written format
  • Utilizes computer as appropriate for communication and reports
  • Follows safety protocols
  • Other duties as assigned

Education and Licensure:

Bachelor's Degree Required

LSW Preferred


  • 2 years of experience working with children and families
  • Knowledge of community resources and comfortability in the home setting
  • Passion for working with families in the community
  • Experience working with a diverse client population in an urban setting
  • Skilled in using computer software applications such as Microsoft Office
  • Excellent verbal and written communication skills
  • Strong organizational and time management skills
  • Must have a valid OH driver's license with no more than 5 points
  • Ability to work a rigorous schedule to handle crisis situations

Preferred Qualifications:

  • Prior community based case management experience
  • Experience working with clients in inner city homes
